Job Overview:

As part of RWJBarnabas Population Health team, you will play an instrumental role in providing fully coordinated patient care from a holistic approach. The Care Coordination Specialist is responsible for facilitating care components such as closing care gaps and preventive care, contributing to quality performance targets for value-based care. You will assist in delivering cost-effective healthcare services throughout the care continuum and serve as a liaison among patients, care teams, physicians, payers, and other healthcare participants.

Qualifications:

Required:

  • Associate's Degree or equivalent work experience
  • 4 years of relevant experience
  • Valid NJ Driver's License or ID and dependable transportation (must obtain within 30 days if not already held)
  • Medical Terminology knowledge
  • Understanding of HIPAA and patient information protection

Preferred:

  • Bachelor's Degree
  • MA or LPN license in NJ
  • Knowledge of population health
  • Experience with Epic or similar EMR systems
  • Bilingual abilities

Schedule Requirements:

  • Travel approximately 3-4 days per week within Ocean, Monmouth, Burlington, and Mercer counties.

Essential Functions:

  • Travel to practices across NJ for EMR reviews, data abstraction, education, and outreach.
  • Facilitate data exchange to monitor quality metric gap closure.
  • Share quality gap reports with practice management.
  • Perform EMR chart reviews, data abstraction, and reporting.
  • Identify potential diagnosis codes and collaborate with Practice Optimization Specialists.
  • Manage relationships to meet needs effectively.
  • Handle high workloads with accuracy and integrity.
  • Perform clinical assessments to close quality gaps.
  • Build partnerships with practices and understand EMR systems.
  • Act as a patient advocate and improve health outcomes.
  • Provide excellent customer service and compassion.
  • Navigate insurance portals and meet quality goals.
  • Perform other duties as assigned.
