Responsibilities

• Provides outreach and initial contact to potential clients to introduce them to the program and engage them in care coordination services.
• Maintains individual case records and protects confidential information. Documents services provided per established guidelines.
• Coordinates an individualized plan of care with the client and family; works with primary care providers and other health care team members to ensure coordination of services.
• Serves as a client advocate with local community agencies and facilitates access to resources related to chronic disease management.
• Works in a fast-paced environment with critical thinking, decisive judgement, and minimal supervision.

Education
