Job summary

Healthfirst, Inc. is seeking a Care Coordinator to support NorthStar Members by managing care navigation, answering benefit questions, and coordinating with care teams.

You will document calls in a PHI system and handle a caseload for short-term care coordination, ensuring timely communication of member needs. The role requires CMA/CNA/RMA or related training, a HS diploma or GED, and strong verbal/written communication skills in a fast-paced environment.

Qualifications

• CMA/CNA/RMA or relevant medical training
• HS diploma/GED
• Excellent verbal and written communication with strong grammar
• Experience in a fast-paced environment handling multiple priorities

Responsibilities

• Receive calls from members about benefits, care coordination, program inquiries, connecting to care manager, and screening assessments to identify risk factors.
• Document calls contemporaneously in the designated PHI system.
• Manage an assigned member caseload requiring short-term care coordination and health navigation.
• Collaborate with the interdisciplinary care team to communicate member needs and monitor screenings.
• Conduct outreach calls to confirm services are in place.
• Perform additional duties as assigned.
