Job summary

Leon Health, Inc. is seeking a Case Coordinator to work under the Director of Health Services, collaborating with a plan Case Manager (RN) on ongoing assessment, planning, and evaluation of members in the Care Management Program.

The role acts as a liaison between patients, families and healthcare personnel to ensure timely and effective care. The Case Coordinator will oversee non-clinical and administrative staff, coordinate with the interdisciplinary care team, and support transitions,

Responsibilities

• Oversee non-clinical and administrative staff
• Coordinate MOC activities, including processing HRAs and individualized care plans
• Collaborate with the interdisciplinary care team for care transitions
• Coordinate with plan RN for high-need D-SNP members
• Identify member needs and create care plans with input from the team
• Support ongoing member engagement and education on conditions
• Promote medication adherence and self-management
• Engage in discharge planning and timely post-discharge services
• Link members to community supports and resources
• Communicate with caregivers, providers, and the ICT about needs and transitions
• Participate in Quality Improvement and Risk Management activities
• Maintain confidentiality and comply with policies
