Qualifications

• Must reside in Illinois.
• Must have reliable transportation and be willing to travel up to 30-40% of the time.
• Strong computer literacy including Excel and Word.
• Able to work independently and in a multidisciplinary team.

Responsibilities

• Evaluate members’ needs/eligibility and recommend an approach to case resolution.
• Coordinate and implement assigned care plan activities.
• Identify high risk factors and refer to clinical case management or crisis intervention as appropriate.
• Provide coaching and support to empower member health decisions.
• Monitor, document, and ensure regulatory and accreditation compliance.
