Job summary

Westbrook Health Services is seeking a Care Coordinator to secure, coordinate, and monitor client services per the Individual Treatment Plan or emergent needs. You will coordinate care across settings, document outcomes, and support transportation and referral activities.

The role emphasizes team-based planning, client-centered care, and ongoing follow-up to ensure access to needed services while maintaining confidentiality and complying with applicable regulations.

Qualifications

• Clear and accurate communication with a variety of individuals.
• Ability to work well in a team environment.
• Professional, cooperative, ethical, respectful interaction with staff, clients and others.
• Ability to engage with a diverse group of people.
• Must have a valid driver’s license and clean driving record.

Responsibilities

• Act as primary contact with client.
• Coordinate and chair Interdisciplinary Team; develop and review Individual Treatment Plans.
• Provide person- and family-centered care.
• Advocate for and assist clients in obtaining needed services (physical, behavioral, social).
