Zusammenfassung

SSM Health Bridgeton McKelvey seeks a detail-oriented chart reviewer to support high-complexity and population health programs in a hybrid setting. You will ensure accurate coding for diagnoses, aid cost-effective care via telephonic outreach, and act as a liaison among patients, physicians, and the care team.

You will review charts, obtain outside records, schedule follow-ups, and help close care gaps while maintaining safety and alertness in a dynamic health system environment.

• Standard work hours are Monday through Friday, 8:00 AM to 4:30 PM.
• Proficiency in using various computer programs to complete work tasks.

Job Summary:

Responsible for detailed preview of charts for high complexity patients and patients in the population health programs within the virtual care center. Works with providers across the organization to ensure accurate coding for diagnosis. Assists in providing cost-effective healthcare services through telephonic outreach. Serves as a liaison between patients, physicians and all other members of the interdisciplinary health care team.

• Bachelor’s degree in a healthcare related field or equivalent combination of education and experience

PHYSICAL REQUIREMENTS

• Constant standing and walking.
• Frequent use of hearing and speech to share information through oral communication.
• Frequent use of hearing to distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
• Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
• Frequent reaching, gripping, and keyboard use/data entry.
• Frequent use of vision for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
• Occasional bending, stooping, crawling, kneeling, sitting, squatting and repetitive foot/leg and hand/arm movements.
• Occasional use of vision to judge distances and spatial relationships.
• Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
• Occasional lifting/moving patients.
• Occasional use of smell to detect/recognize odors.
