Generations Home Care is seeking a Care Coordinator in Kitsap County, WA. This full-time, hourly role requires strong organization, attention to detail, and compassion. You''ll report to the Office Manager and work with healthcare professionals to ensure high-quality client care.

The position offers an hourly rate of $25-$30 with semimonthly pay and a comprehensive benefits package including health insurance, PTO, dental/vision, 401k, and professional development opportunities.

Qualifications

• High school diploma or equivalent; college degree preferred.
• 2 years of experience in home care or related healthcare field.
• Strong organizational and time-management skills.
• Excellent communication and interpersonal skills.
• Compassionate, patient, and empathetic.
• Ability to work independently and as part of a team.
• Ability to learn new technology and use scheduling software (WellSky).
• Valid driver''s license and reliable transportation.
• Able to pass a background check and drug screening.

Responsibilities

• Coordinate and schedule care services for clients based on their needs.
• Update care plans with the Care Coordination Team.
• Communicate with clients and families to assess needs and provide support.
• Maintain records of client care, progress, and changes.
• Collaborate with nurses and therapists for coordinated care.
• Assist with recruiting, training, and supervising care staff.
• Participate in team meetings and share client care updates.
• Address client or staff concerns promptly.
• Stay current on home care industry trends and best practices.
