Job summary

UCHealth in Colorado/Aurora is seeking a full-time Care Coordination Specialist to support outreach, care coordination, and transitions across pediatric to adult care. The role collaborates with primary care teams and community resources to ensure seamless service delivery and patient engagement.

The position offers hybrid or remote options, a competitive pay range of $25.80 to $38.70 per hour, and a comprehensive benefits program including health/dental/vision coverage and tuition assistance.

Qualifications

  • Bachelor's degree in Public Health, Nutrition, Exercise Science, Health, Wellness, or related field.
  • Experience with underserved populations or in community health programs preferred.

Responsibilities

  • Provide outreach and initial contact to potential clients to engage them in care coordination services.
  • Maintain case records and protect confidentiality; educate clients using population-specific criteria.
  • Coordinate individualized plans of care with clients and families; work with primary care providers to ensure service coordination.
  • Coordinate appointments and transportation resources; conduct home visits or visits at provider sites as needed.
  • Advocate for clients with local community agencies; provide emotional support and education on self-care skills.
  • Facilitate transitions for young adults moving from pediatric to adult healthcare systems; ensure continuity of care.
  • Partner with clinics and community resources to reduce barriers to care.
  • Assist with insurance and social determinants of health navigation; support access to specialty care.
  • Provide care coordination, resource navigation, and family support to promote engagement in adult services.
  • Work in a fast-paced environment with decisive judgment and minimal supervision.

Work Schedule: Full Time, 80.00 hours per pay period (2 weeks)

Shift: Days

Pay: $25.80 - $38.70 / hour. Pay is dependent on applicant's relevant experience

This position will offer a hybrid or remote option

This is a grant-funded position and will be considered for renewal annually

Summary:
  • Serves clients under appropriate supervision and participates in regular case conferences. Provides outreach and initial contact to potential clients to introduce them to the program and engage them in care coordination services.

  • Maintains individual case records and protects confidential information. Provides education to clients using population-specific criteria. Documents services provided per established guidelines.

  • Coordinates an individualized plan of care with the client and family. Works with primary care providers and other members of the health care team to ensure coordination of services. Coordinates appointments with providers, utilizing transportation resources as needed. Meets clients in their home, at a provider's office, in the hospital, or in the community as part of ongoing support, offering monitoring of care. Provides appropriate follow-up.

  • Serves as a client advocate with local community agencies (as permitted by clients), and provides emotional support. Facilitates access to key resources including information and education related to chronic disease management. Educates and coaches clients and caregivers about self care skills; collaborates with providers to achieve desired outcomes.

  • Coordinate transitions for medically and socially complex young adults moving from pediatric to adult healthcare systems, ensuring continuity of care and connection to needed services.

  • Partner with clinics, care teams, and community resources to support seamless care transitions and reduce barriers to care.

  • Address patient needs related to insurance, social drivers of health, transportation, equipment, and navigate access to specialty care.

  • Provide care coordination, resource navigation, and patient and family support to promote successful transition outcomes and engagement in adult healthcare services.

  • Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

Requirements:
  • Bachelor's degree in Public Health, Nutrition, Exercise Science, Health, Wellness, or related field.

  • Preferred: Experience working with under-served populations or in community health programs.
