Job summary

COPE Health Solutions is seeking a Social Worker to join a multidisciplinary team in Riverside, CA, delivering in-person and telephonic support for patient navigation and care coordination. The role emphasizes assessments, care planning, and collaboration with physicians, nurses, and community resources to meet patient needs.

The position requires licensure, 3–5 years in care management, and travel up to 75%. Exempt, full-time with a competitive salary and benefits package.

Qualifications

• Valid and current social work licensure (LMSW/LCSW) is required.
• Experience with care management in healthcare settings is preferred.
• Proficient in EMR systems, Outlook, and data entry.

Responsibilities

• Assess members early in disease process to identify management needs.
• Develop care plans using clinical judgment and available services.
• Assess Social Determinants of Health ( housing, food, transportation, safety ).
• Collaborate with physicians, nurses, pharmacists and other teams.
• Screen and address behavioral health concerns with appropriate interventions.
• Facilitate referrals and follow-up for behavioral health needs.
• Create and update a multidisciplinary plan of care with family and providers.
• Coordinate referrals/authorizations and track progress toward goals.
• Engage patients in wellness activities and promote appropriate care settings.
• Communicate status with care team and support staff.
