Job summary
The Durham VA Healthcare System is seeking highly skilled Registered Nurses for our Care Coordination Center to lead and manage complex patient care, ensuring a safe and quality-focused environment.

Qualifications
- Requires active US nursing license in good standing.
- Admission to or completion of bridge programs may be required for licensure.
- Strong knowledge of nursing process and care coordination across settings.
- Ability to work with multidisciplinary teams and care transitions.
Responsibilities
- Responsible and accountable for all elements of the nursing process in care coordination.
- Coordinate care for Veterans admitted locally or transitioning between VA and community facilities.
- Lead and participate in quality improvement and customer service initiatives.
- Educate patients and families, support self-management, and ensure positive outcomes.
- Collaborate with VA and non-VA departments to address patient needs.
