You will coordinate patient flow, develop and update care plans, and serve as a clinical resource for nurses and providers. You will triage, participate in discharge planning, ensure regulatory/documentation compliance, mentor staff, and collaborate with physicians, social workers, and ancillary teams to advance quality initiatives in a mission-driven, patient-focused

Formación

• Experience coordinating care across care teams.
• Ability to develop and update comprehensive care plans.
