Job summary

Penn State Health Holy Spirit Medical Center is seeking a Registered Nurse Navigator to guide patients through discharge planning and coordinate care across the outpatient setting.

The RN Navigator will support staff, act as a clinical resource to scheduling coordinators, and suggest alternative care locations when discharge needs arise. An RN license and BLS certification are required.

Qualifications

• Active RN license in the state of employment.
• AHA BLS certification required before orientation.

Responsibilities

• Facilitate the patient journey and ensure proper navigation through the care system upon discharge.
• Coordinate care and support staff during transitions in the outpatient setting.
• Serve as a clinical resource to scheduling coordinators and identify referral options.
