Qualifications

• MSW degree required with WA licensure upon hire.
• Licensed Independent Clinical Social Worker (WA) upon hire.
• Licensed Advanced Social Worker or Licensed Social Worker Associate Advanced in WA upon hire.

Responsibilities

• Provide developmentally appropriate psychosocial care and plan discharge for all ages and conditions.
• Advocate for clients and deliver education and resources.
• Coordinate psychosocial assessment, care planning, and crisis intervention.
• Lead Patient/Family Care Conferences to align care transitions.
• Oversee Continuity/Transition management for patients.
• Coordinate community resources and health-related programs to address social determinants.
