• Familiarity with EMR documentation and regulatory requirements.

Responsibilities

• Assist with discharge planning, referrals, and follow-up services.
• Communicate with insurers to verify coverage and obtain approvals.
• Document care management activities in the EMR with accuracy.
• Support patient and family communication about discharge instructions and resources.
• Coordinate with home health, DME, SNFs and post-acute services to ensure smooth transitions.
