Job summary

Renown Health in Reno seeks a Discharge Planning Assistant to support RN Case Managers and Social Workers in Hospital Care Management and Acute Rehab teams. You coordinate discharge plans, communicate with care team, and help move patients toward timely discharge.

Responsibilities include attending daily rounds, initiating referrals to home care, infusion, wound care, DME, SNF and hospice providers, and documenting activities in the medical record.

Qualifications

• Two years clerical experience in the medical field preferred.
• Must have working-level knowledge of English (reading, writing, speaking).
• Must possess or obtain computer skills within 90 days to complete online learning and access Epic and other applications.

Responsibilities

• Attend unit rounds daily to receive direction and communicate progress of discharge planning activities.
• Initiate referrals and follow-up calls for home care agencies, infusion, wound care providers, DME, SNF and hospice as requested by the care team.
• Document all discharge planning activities appropriately in the medical record.
