Qualifications

• Bachelor's degree or higher in social work, nursing or human services.
• LSW or RN preferred.

Responsibilities

• Collaborates with members of the health care team.
• Oversees communication of the plan of care to patient/family, physicians and other healthcare team members.
• Demonstrates understanding of care management philosophy and principles through problem-solving and critical thinking to facilitate positive outcomes.
• Participates in PVH committees.
• Ensures assessments are holistic addressing physical and psychosocial health, environment, and resources.
• Assists in discharge planning and coordination of post-discharge services.
