Job summary

CommonSpirit Health's Saint Joseph Hospital in Lexington, KY, is seeking a Care Coordination Assistant to serve as a central hub for patient care processes across all ages. You will support safe, therapeutic care while acting as the primary receptionist and secretary on the unit.

You will collaborate with care team members to manage post-acute referrals, coordinate transitions, and ensure follow-up appointments, with emphasis on readmission prevention and continuity of care.

Qualifications

  • Two-year Associates degree in health-related field or equivalent.
  • High school diploma or GED required.
  • Preferred: 3+ years in healthcare, care coordination, discharge planning or similar setting.

Responsibilities

  • Collaborate with the Care Coordination team (RNCC and Social Worker).
  • Manage post-acute care referrals for length of stay management and denials mitigation.
  • Coordinate with interdisciplinary teams, patients and family to facilitate care progression.
  • Ensure post-acute follow-up appointments to aid readmission prevention and continuity of care.
  • Deliver required patient notifications and obtain signatures to meet regulatory requirements.
  • Arrange transportation referrals as requested.

Job Summary and Responsibilities

As our Care Coordination Assistant, you will be a vital caregiver and the central hub, assuming responsibility for patient care processes from newborn to geriatric for a designated time frame. You will directly contribute to safe, therapeutic patient care while serving as the primary receptionist and secretary.

Every day you will support safe patient care through multidisciplinary processes and coordinating unit activities. You will efficiently manage information flow and assist healthcare team members in delivering care across all patient age groups.

To be successful in this role, you will demonstrate strong organizational and communication skills, a proactive approach to managing day-to-day operations, and a commitment to providing safe, therapeutic care while working effectively with the entire healthcare team.

  • Collaborates with and supports the Care Coordination team, including, but not limited to the RNCC and Social Worker.
  • Manage timely post-acute care referrals, to assist with length of stay management and mitigation of denials.
  • Communicate and collaborate with interdisciplinary teams, patients, and family to facilitate care progression.
  • Ensure post-acute follow-up appointments for identified patients to assist with readmission prevention and ensuring continuity of care.
  • Deliver required patient notifications and letters, and obtain patient or family signatures on required documentation as needed to meet regulatory compliance.
  • Manage transportation referrals as requested.
Job Requirements

Required

  • Associates Other Associate (two-year) Degree in Business/Health-Related field, upon hire or

  • At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting. and
  • Bachelors Other or BSW, upon hire and
  • Experience supporting clinicians in demanding and fast paced patient care environments.
  • Licensed Practical Nurse: KY, upon hire or
  • Certified Clinical Medical Assistant, upon hire or
  • Certified Medical Assistant AAMA, upon hire or
  • Certified Medical Assistant NHA, upon hire or
  • National Certified Medical Assistant NCCT, upon hire or
  • National Registered/Certified Medical Assistant NAHP, upon hire or
  • Registered Medical Assistant AMT, upon hire or
  • Registered Medical Assistant, upon hire or
  • Cert Med Asst - MMCI, upon hire or
  • Certified Nursing Assistant: KY, upon hire or

Pay Range

$17.24 - $24.35 /hour
