Qualifications

  • Associate Degree in Business/Health-Related field or High School Diploma with current healthcare license/certification (MA/CNA/EMT).
  • Medical terminology certificate or ability to demonstrate knowledge of medical terminology.

Responsibilities

  • Communicate and collaborate with interdisciplinary teams, patients, and families to facilitate care progression.
  • Manage timely post-acute care referrals to assist with length of stay management and mitigation of denials.
  • Ensure post-acute follow-up appointments for identified patients to aid readmission prevention and continuity of care.
  • Prioritize, document, and transmit communications relating to insurance requests, authorizations, denials and Physician Advisor status to the appropriate licensed staff.
  • Comply with confidentiality policies, HIPAA regulations, and organizational standards when transmitting protected patient information to external agencies and vendors.
  • Collaborates with and supports the Care Coordination team, including RN/CC and Social Worker.

Job Summary and Responsibilities

As our Care Coordination Assistant (CCA), you will support critical and time-sensitive functions of the Clinical Care Coordination team, managing post-acute care referrals and communications with patients, families, and agencies.

Every day you will perform duties with a high degree of accuracy and critical thinking, including direct communication with key stakeholders, ensuring compliance with hospital policies, standards of practice, and Federal/State regulations, all while upholding our organizational values.

To be successful in this role, you will demonstrate strong organizational skills, meticulous attention to detail, effective communication abilities, and a commitment to accuracy and compliance in supporting comprehensive care coordination.

  • Communicate and collaborate with interdisciplinary teams, patients, and family to facilitate care progression.
  • Manage timely post-acutecare referrals to assist with length of stay management and mitigation of denials.
  • Ensure post-acutefollowup appointments for identified patients to assist with readmission prevention and ensuring continuity of care.
  • Prioritize, manage, and document communications relating to insurance requests, authorizations, denials and Physician Advisor status recommendations to the appropriate licensed staff member as applicable.
  • Comply with confidentiality policies, HIPAA regulations, and organizational standards when transmitting protected patient information to external agencies and vendors as needed for placement, payment or referral.
  • Collaborates with and supports the Care Coordination team, including, but not limited to the RNCC and Social Worker.

Schedule: Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of weekend rotation

Job Requirements

Required

  • Associate Degree in Business/Health-Relatedfield or High School Diploma or GED with a current license/certification as a healthcare professional (Medical Assistant/Certified Nursing Assistant/Emergency Medical Technician).
  • Medical terminology certificate or ability to demonstrate knowledge of medical terminology.

Preferred

  • Three (3) or more years experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting.
  • Bachelor Degree or BSW.
  • Experience supporting clinicians in demanding and fast paced patient care environments.
  • LVN/LPN, Licensed BSW, Medical Assistant, Certified Nursing Assistant, or Emergency Medical Technician certificate.
  • Awareness of CMS and other regulatory guidelines.
