Job summary

St. Anthony Hospital in Lakewood, CO seeks a Care Coordination Assistant to support the Clinical Care Coordination team.

You will manage post-acute care referrals, communicate with patients and families, and coordinate with post-acute care agencies for timely transitions. The role emphasizes compassionate, compliant care in a fast-paced hospital setting with a schedule of Monday to Friday, 8:00 AM to 4:30 PM, and potential weekend rotation.

Qualifications

  • High School Diploma/GED with a healthcare license/certification or an associate degree in a health-related field.
  • Medical terminology knowledge or ability to demonstrate knowledge is required.
  • Preferred: 3+ years in health care, care coordination, discharge planning or similar setting.

Responsibilities

  • Support post-acute care referrals and direct communication with patients and families.
  • Coordinate with post-acute care agencies and other stakeholders.
  • Ensure timely, accurate care transitions in compliance with policies and regulations.
  • Assist care teams with data and documentation as needed.

The Care Coordination Assistant role is responsible for supporting critical and time sensitive functions of the Clinical Care Coordination team, including but not limited to managing post acute care referrals, direct communication with patients/families, post acute care agencies, and other key stakeholders. The Care Coordination Assistant performs these duties with a high degree of accuracy utilizing critical thinking skills and in compliance with hospital policies, standards of practice, and Federal and State regulations. Schedule:

Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of weekend rotation

Job Requirements

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

Required
  • High School Diploma/GED with a current license/certification as a healthcare professional (Medical Assistant/Certified Nursing Assistant/Emergency Medical Technician) or Associate (two-year) Degree in Business/Health-Related field
  • Medical terminology certificate or ability to demonstrate knowledge of medical terminology is required.
Preferred
  • At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting.
  • Bachelor’s degree or BSW.
  • Experience supporting clinicians in demanding and fast paced patient care environments.
Physical Requirements
  • Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
