Job summary

Mercy Gilbert Medical Center is seeking a Care Coordination Assistant to support the Care Coordination team in delivering safe, therapeutic patient care across age groups. You will serve as the primary receptionist and secretary, coordinating unit activities and information flow to support multidisciplinary teams.

  • Intermediate to advanced computer skills including Google Workspace (Docs, Sheets) typing and data entry.

Responsibilities

  • Collaborates with and supports the Care Coordination team, including RN Care Coordinator and Social Worker.
  • Manage timely post-acute care referrals for length of stay management and denials mitigation.
  • Communicate and collaborate with interdisciplinary teams, patients, and family to facilitate care progression.
  • Ensure post-acute follow-up appointments for identified patients to aid readmission prevention and continuity of care.
  • Deliver patient notifications and letters and obtain signatures to meet regulatory compliance.

Job Summary and Responsibilities

As our Care Coordination Assistant at Chandler Regional Medical Center, you will be a vital part of the Care Coordination team, assuming responsibility for patient care processes from newborn to geriatric for a designated time frame. You will directly contribute to safe, therapeutic patient care while serving as the primary receptionist and secretary. Every day you will support safe patient care through multidisciplinary processes and coordinating unit activities. You will efficiently manage information flow and assist healthcare team members in delivering care across all patient age groups. To be successful in this role, you will demonstrate strong organizational and communication skills, a proactive approach to managing day-to-day operations, and a commitment to providing safe, therapeutic care while working effectively with the entire healthcare team.

  • Collaborates with and supports the Care Coordination team, including, but not limited to the Registered Nurse Care Coordinator and Social Worker.
  • Manage timely post-acute care referrals, to assist with length of stay management and mitigation of denials.
  • Communicate and collaborate with interdisciplinary teams, patients, and family to facilitate care progression.
  • Ensure post-acute follow-up appointments for identified patients to assist with readmission prevention and ensuring continuity of care.
  • Deliver required patient notifications and letters, and obtain patient or family signatures on required documentation as needed to meet regulatory compliance.

Required
  • Associate (two-year) Degree in Business/Health-Related field or High School Diploma or GED
  • Medical terminology certificate or ability to demonstrate knowledge of medical terminology
  • Intermediate to advanced computer skills required to include Google Workspace applications including Docs, Sheets, typing, and data entry skill
Preferred
  • At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting
  • Bachelor’s degree or BSW
  • Experience supporting clinicians in demanding and fast paced patient care environments
