Job summary

Baptist Health is seeking a Follow Up Care Coordinator to support post-encounter care across our Northeast Florida network. The role is full-time, hybrid, located at Southbank, with scheduling responsibilities in EPIC for 19 BPC clinics and liaison work with physicians and care teams.

You will build relationships, inform patients about resources, participate in programs like New Mover's and PATH, and help ensure exceptional customer service with QA-driven metrics.

Qualifications

• 1-2 years Database Maintenance experience.
• 1-2 years Customer Service experience.

Responsibilities

• Assist patients post-encounter at Baptist Free Standing ED or CareSpot with follow-up care needs.
• Inform and educate Baptist Health resources, services, providers and facilities.
• Support New Mover's Program, PATH, and Women’s Health Strategy initiatives.
• Direct scheduling within EPIC for 19 BPC clinics.
• Act as liaison between patients and clinic offices on service line initiatives.
• Provide concierge, personalized health care coordination with exceptional customer service.
