Care Companion/Social Worker

We are seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team at Carease Health - MA. As a Care Companion/Social Worker, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

About the Role

At Carease Health, we prioritize proactive measures for enhanced physical health and well-being by incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program. Our experienced clinical team operates discreetly in the background, seamlessly aligning with your facility's processes and protocols to ensure minimal disruption to your current operations.

Key Responsibilities

  • Visit designated facilities in accordance with the schedule set with your supervisor

  • Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs

  • Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns

  • Document visit notes and maintain detailed records of interactions and interventions after each visit

  • Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications

  • Associate, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field

  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

  • Strong communication, documentation, and relationship-building skills

  • 1 year of experience
