Carease Health - PA is seeking a compassionate Mental Health Provider as a Care Companion/Social Worker to support residents in skilled nursing facilities. You will offer emotional support, brief therapy, and timely mental health assessments, contributing to their well-being and sense of connection.

The role involves facility visits, collaborative care with psychiatric professionals, and ongoing documentation.

Formación

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.

Responsabilidades

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
