Job summary

Carease Health is seeking a compassionate Mental Health Provider to join our Collaborative Care team as a Care Companion. You will offer emotional support, brief therapy, and mental health assessments to residents in skilled nursing facilities, positively impacting older adults’ lives.

You will visit facilities on a set schedule, conduct assessments, deliver short-term therapy, and document interactions. Collaboration with psychiatric professionals and monthly meetings with the psychiatric NP

Qualifications

• Degree in social work, psychology, counseling or related field.
• Geriatric mental health understanding and crisis management.
• Strong communication, documentation and relationship-building skills.
• Experience: 1 year in a mental health setting.

Responsibilities

• Visit designated facilities per supervisor schedule.
• Assess residents' emotional, cognitive and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain records after each visit.
• Collaborate with psychiatric professionals and attend monthly meeting with the psychiatric NP.

Job Summary

Key Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.

Qualifications

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree
• Experience: 1 year
