Job Title: Care Center Registered Nurse (RN)
Job Location: Livonia, MI 48154
Position Type: Full Time
Salary: $36.00 hourly

Role Summary

The Care Center RN provides hospice-skilled professional nursing care as prescribed by the physician and in compliance with the medical plan of care. The RN works as part of an interdisciplinary team and follows the philosophy, goals, and objectives of Angela Hospice and Felician Franciscan Core Values.

Responsibilities
  • Perform ongoing comprehensive assessments of patients and families to determine hospice needs.
  • Provide professional nursing care using specialized end-of-life skills.
  • Initiate and revise the patient’s plan of care, involving the patient and family in planning.
  • Administer or order medications and treatments, and document all interventions.
  • Educate patients, families, and caregivers about medications, treatments, disease processes, and end-of-life care.
  • Communicate patient/family concerns to interdisciplinary team members and initiate corrective actions.
  • Assist patients with activities of daily living, promote self-sufficiency and comfort care.
  • Transport and lift patients safely using proper body mechanics or devices.
  • Maintain accurate clinical notes, pain and symptom management records, and communicate changes to physicians or nurse practitioners.
  • Collaborate with community health professionals to coordinate the care plan.
  • Teach self-care techniques to patients and families/caregivers.
  • Maintain a safe environment, ensure privacy and dignity, and provide compassionate care.
  • Report incidents or abuse to the Lead Nurse, Director of Inpatient Services, or administrative staff.
  • Provide clinical supervision to LPNs, SNAs, hospice aides, and volunteers, delegating responsibilities when needed.
  • Ensure availability of necessary equipment and services.
  • Engage in quality improvement initiatives and comply with Medicare, Medicaid, HIPAA, and state regulations.
  • Participate in continuing education, seminars, conferences, and in-service training.
  • Support department operations and special projects as directed.
  • Attend interdisciplinary group meetings as required.
  • Maintain professional knowledge and practice skills through ongoing education.
  • Achieve and maintain compliance with Federal/State regulatory guidelines.
  • Perform all other duties as requested by the President/CEO, Director of Inpatient Services, or other designee.
Supervisory Responsibility

Supervises LPNs, SNAs, hospice aides, and volunteers; mentors staff and volunteers and orients new employees.

Physical Requirements

Requires regular standing, walking, and lifting of a minimum of 50pounds. Must be able to bend, stoop, turn, and twist. Requires use of a computer, phone, and other office equipment.

Work Environment

Health-care setting with direct patient and team member interaction. Includes walking to various areas of the building.

Expected Hours

Position is open 24/7, 365days a year. Scheduled regular, additional, holiday shifts and work during inclement weather may be required.

Travel

Not routinely required but may be necessary as needed.

Qualifications
  • Graduate of an accredited nursing school.
  • Current licensed RN in the State of Michigan.
  • CPR certified.
  • Hospice experience in an inpatient or home-care setting preferred.
  • Excellent organizational and time-management skills.
  • Proficient computer and phone skills.
  • Strong observation, verbal, and written communication skills.
  • Problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Capacity to work with minimal supervision.
  • Ability to lift, reposition, and transfer patients.
  • Auditory and visual acuity sufficient for patient care.
  • Proficient interpersonal and communicative skills.
  • Knowledge of Federal/State regulations, NHPCO, and CHAP guidelines.
