Community Care is hiring a Care/Case Manager. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position located in Waukesha.
Care/Case Manager Job Responsibilities:
Care/Case Manager Job Requirements:

Education: Bachelor’s Degree in a Human Services or closely related a rea required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.

Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred .

Certification: Eligibility for long-term care functional screener certification is required
·
Travel : Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
