Job summary

Optum in Mississippi is seeking a nurse practitioner for in-home care travels across the Gulfport area. No nights, weekends or holidays; daily travel to patient homes within assigned area.

Compensation includes a $10,000 sign-on bonus or $20,000 loan repayment and a comprehensive benefits package. Responsibilities include in-home assessments, medical history, vitals, exams, disease management, care planning, and counseling for patients and families.

Qualifications

• ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted driver's license with reliable transportation.
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive in-home assessments including past medical history, medications, vitals and physical exams.
• Perform evidence-based screenings and point-of-care testing as appropriate.
• Manage patients through acute events or chronic conditions and sub-acute rehab episodes.
• Ensure accurate documentation including ICD-10 codes and care plans.
• Collaborate with interdisciplinary care team and discuss advanced illness and care planning.
• Provide patient and caregiver counseling and education.

Primary Responsibilities:

• Conduct comprehensive in-home assessments, including:
• Past medical history review, medication reconciliation, vital signs, and physical exam
• Evidence-based screenings and point-of-care testing (as appropriate)
• Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
• Manage members during a short term sub-acute rehabilitation episode
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education

Required Qualifications:

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver's license
  • In addition to a driver's license, access to reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
