UnitedHealth Group seeks a nurse practitioner to conduct in-home assessments, reviews meds, and performs exams with essential point-of-care testing. The role includes managing acute events and rehab episodes, while documenting ICD-10 conditions and coordinating with a care team.

Must hold an active NP certification (ANCC/AANP), an unrestricted license, and a valid driver’s license with reliable transportation. Sign-on bonus is available; travel is required within the assigned area.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in the practicing state
• Active and unrestricted driver's license with reliable transportation
• Current DEA licensure/prescriptive authority or ability to obtain post-hire
• Ability to establish a collaborative practice agreement and navigate varied environments

Responsabilités

• Conduct comprehensive in-home assessments including medical history, meds, vitals and exam
• Perform evidence-based screenings and point-of-care testing as appropriate
• Manage patients through acute events and chronic condition exacerbations
• Support short-term sub-acute rehabilitation and ensure proper documentation
• Communicate with the interdisciplinary care team and provide patient education
• Discuss advanced care planning and counsel patients and caregivers

$10,000 Sign-on Bonus or $20,000 loan repayment. In Home Care, No Nights, Weekends or Holidays. This position requires daily travel to patient homes within assigned area.

Primary Responsibilities:

• Conduct comprehensive in-home assessments, including:
• Past medical history review, medication reconciliation, vital signs, and physical exam
• Evidence-based screenings and point-of-care testing (as appropriate)
• Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
• Manage members during a short term sub-acute rehabilitation episode
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education

Required Qualifications:

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver's license
  • In addition to a driver's license, access to reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
