Avera at Home Milbank is hiring a PRN caregiver to provide personal care for elderly, convalescent or disabled individuals in home or facility settings. You will work as part of a multidisciplinary team to deliver compassionate, patient-centered care following the established plan.

The role requires a CNA with a valid driver’s license; high school diploma is preferred. Mileage reimbursement is provided, and you will document care activities and communicate changes to the supervising nurse.

Formación

• Driver's License required; Licensing Board Upon Hire.
• Certified Nurse Aide (CNA) required; license in state of practice may be required upon hire.
• High School diploma or GED preferred; active CNA license preferred.

Responsabilidades

• Assist patients with activities of daily living.
• Provide personal care as directed by the nurse and plan of care.
• Perform housekeeping, laundry, meal preparation, and shopping.
• Assist patients with social activities as prescribed by the plan of care.
• Maintain timely and accurate documentation.
• Report changes in patient status to supervisor.
• Demonstrate care appropriate to the age of the patient served.
• Use equipment and supplies effectively and efficiently.
• Adhere to safety, infection control, and environmental policies.

A Brief Overview

Provides personal care for elderly, convalescent or disabled persons in home or facility settings. Demonstrates the knowledge and skills necessary to provide care for the population served by Avera.

What You Will Do

• Provides assistance to patients with their activities of daily living.
• Provides personal care for patients as directed and delegated by the nurse, according to the established plan of care.
• Provides services such as housekeeping, laundry, meal preparation, and shopping.
• Assists patient with social activities as prescribed by the plan of care.
• Maintains timely and accurate documentation.
• Reports changes in patient status to supervisor.
• Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient served in the agency.
• Uses equipment and supplies effectively and efficiently.
• Adheres to environmental, safety and infection policies and procedures.

Required Education, License/Certification, Or Work Experience

• Drivers License - Licensing Board Upon Hire
• Certified Nurse Aide (CNA) - Licensing Board If working in the state of Minnesota Upon Hire

Preferred Education, License/Certification, Or Work Experience

• High School or GED Equivalent
• Certified Nurse Aide (CNA) - Licensing Board An active license in the state of practice
• Less than 1 year in home health care or related health care field
