Job summary

Sioux Center Health seeks a Certified Nurse Aide to provide personal care for elderly, convalescent or disabled persons in home or facility settings. You will deliver care per the established plan and assist with daily living activities under nurse direction.

The role requires CNA certification, a valid driver's license, and ability to work day shift with 72 hours/pay period. PTO begins day 1; student loan repayment may be available; join a multidisciplinary team dedicated to compassionate care.

Qualifications

• Visual acuity adequate to perform position duties and communicate effectively.
• Able to assist with activities of daily living and provide personal care as directed.
• Must be able to operate in home or facility settings.

Responsibilities

• Provides assistance to patients with activities of daily living.
• Provides personal care for patients as directed and delegated by the nurse.
• Performs housekeeping, laundry, meal preparation, and shopping.
• Assists patient with social activities as prescribed by the plan of care.
• Maintains timely and accurate documentation; reports changes to supervisor.

What You Will Do

• Provides assistance to patients with their activities of daily living.
• Provides personal care for patients as directed and delegated by the nurse, according to the established plan of care.
• Provides services such as housekeeping, laundry, meal preparation, and shopping.
• Assists patient with social activities as prescribed by the plan of care.
• Maintains timely and accurate documentation.
• Reports changes in patient status to supervisor.
• Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient served in the agency.
• Uses equipment and supplies effectively and efficiently.
• Adheres to environmental, safety and infection policies and procedures.

The individual must be able to work the hours specified. To perform this job successfully, an individual must be able to perform each essential job function satisfactorily including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand and distinguish speech and other sounds. Required Education, License/Certification, Or Work Experience

• Drivers License - Licensing Board Upon Hire
• Certified Nurse Aide (CNA) - Licensing Board If working in the state of Minnesota Upon Hire

Preferred Education, License/Certification, Or Work Experience

• High School or GED Equivalent
• Certified Nurse Aide (CNA) - Licensing Board An active license in the state of practice
• Less than 1 year in home health care or related health care field
