Job summary

Avera at Home Pierre is seeking dedicated aides to provide compassionate personal care in home settings. You will assist with daily living activities, support nurses, and help with household tasks while maintaining accurate documentation.

The role emphasizes teamwork, safety, and a patient-centered approach to aging and disability care. PTO applies, with a sign-on possibility for eligible hires.

Qualifications

• Required CNA certification and an active license to practice in the relevant state.
• Driver's license required for client visits.
• High school diploma or GED acceptable per preferences.

Responsibilities

• Provides assistance to patients with activities of daily living.
• Delivers personal care as directed by the nurse and plan of care.
• Performs housekeeping, laundry, meal prep and shopping as needed.
• Assists with social activities per the plan of care.
• Maintains timely and accurate documentation.
• Reports changes in patient status to supervisor.
• Utilizes equipment and supplies safely and efficiently.
• Adheres to safety, infection control and agency policies.
• Works within a multidisciplinary team to move health forward.

A Brief Overview

Provides personal care for elderly, convalescent or disabled persons in home or facility settings. Demonstrates the knowledge and skills necessary to provide care for the population served by Avera.

What You Will Do

• Provides assistance to patients with their activities of daily living.
• Provides personal care for patients as directed and delegated by the nurse, according to the established plan of care.
• Provides services such as housekeeping, laundry, meal preparation, and shopping.
• Assists patient with social activities as prescribed by the plan of care.
• Maintains timely and accurate documentation.
• Reports changes in patient status to supervisor.
• Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient served in the agency.
• Uses equipment and supplies effectively and efficiently.
• Adheres to environmental, safety and infection policies and procedures.

The individual must be able to work the hours specified. To perform this job successfully, an individual must be able to perform each essential job function satisfactorily including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand and distinguish speech and other sounds. Required Education, License/Certification, Or Work Experience

• Drivers License - Licensing Board Upon Hire
• Certified Nurse Aide (CNA) - Licensing Board If working in the state of Minnesota Upon Hire

Preferred Education, License/Certification, Or Work Experience

• High School or GED Equivalent
• Certified Nurse Aide (CNA) - Licensing Board An active license in the state of practice
• Less than 1 year in home health care or related health care field
