You’ll support families through aging-related transitions while also representing Appian across our trusted network of providers and professionals.

What You’ll Do

• Build and nurture relationships as the family’s trusted advocate, relationship manager, and primary point of contact
• Provide holistic assessments across medical, residential, legal/financial, and lifestyle domains
• Create customized action plans, guiding families through sensitive or complex decisions
• Project manage the execution of action plans and coordinate with external providers
• Refer clients to trusted experts in our curated network
• Identify and build relationships with new strategic partners in the aging and healthcare space
• Represent Appian at select meetings, events, and networking opportunities
• Maintain clear, detailed documentation of client interactions and partner activity
  

What Makes You a Great Fit

• Background in elder care, healthcare, or aging services — or relevant personal life experience
• (Ideal profiles: Social Work, Nursing, OT/PT, Care Management, Health Coaching, Senior Living roles)
• Warm, intuitive communicator with excellent listening skills
• Skilled at holding space for emotionally sensitive conversations
• Organized, professional, and confident navigating both client and partner relationships
• Mission-driven and proactive — comfortable in a startup environment
• Tech-comfortable and detail-oriented
• Based in the Los Angeles area (required)
