Responsibilities

• Administer medications and deliver individualized care in collaboration with the care team.
• Coordinate care plans and support safe transitions with physicians and multidisciplinary colleagues.
• Educate patients and families to promote understanding, adherence, and strong outcomes.
• Use advanced medical technology and participate in continuous improvement efforts.
