Qualifications

• Must be a licensed RN with active practice in the state or compact license.
• Minimum 3 years adult acute care clinical experience with at least 2 years in cardiology.
• Certifications required upon hire unless otherwise stated.
• Experience in case management and remote patient monitoring preferred.
• Proficiency in electronic medical records and documentation.

Responsibilities

• Monitors data output from implanted remote monitoring devices and coordinates care with providers.
• Performs initial assessment and assists with care coordination across inpatient and outpatient settings.
• Serves as liaison between patient/family and healthcare providers.
• Coordinates care across the continuum to ensure successful transitions and care access.
• Assists in clinical guidance development and participates in improvement projects.
