AdventHealth Medical Group Cardiothoracic Surgery at Celebration is seeking an experienced APRN or PA-C to join a collaborative transplant team. You’ll deliver comprehensive pre- and post-transplant care in outpatient and inpatient settings across AdventHealth Celebration and AdventHealth Orlando, under physician supervision.

You will educate patients and families, coordinate complex care with the transplant team, and participate in day-shift coverage with no overnight call.

Qualifications

• Previous cardiothoracic or CT Surgery experience.
• Ability to provide care in both outpatient and inpatient environments.
• Strong clinical judgment and confidence managing cardiothoracic surgery patients.
• Collaborative mindset and commitment to team-based care.
• Strong communication and care-coordination skills.
• Dedication to compassionate, whole-person care.
• This opportunity is intended for an experienced APP and is not currently open to new graduates.

Responsabilités

• Provide comprehensive care for patients throughout the heart and lung transplant journey, including pre-transplant evaluation and post-transplant follow-up.
• Care for patients with advanced and end-stage heart and lung disease.
• Support ongoing management and surveillance of patients following heart or lung transplantation.
• Collaborate closely with the transplant team to coordinate complex, multidisciplinary patient care.
• Manage patients across both outpatient and inpatient settings, supporting continuity of care throughout the transplant journey.
• Educate patients and families regarding treatment plans, medications, recovery, and long-term care.
