Job summary

ProMedica in Toledo, OH is seeking an RN Program Coordinator for the Cardiology department to ensure collaborative care from hospital discharge through follow-ups. You will coordinate patient plans of care, provide education for procedures and testing, and review Health Risk Assessments with providers.

In this role you monitor patients' charts for test results, review preoperative surgical clearance with providers, and may triage calls or provide health advice and referrals as needed.

Qualifications

• Must hold a current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification required.

Responsibilities

• Coordinate patient care plans from discharge through follow-ups.
• Provide education/instructions for procedures and testing ordered by providers.
• Review Health Risk Assessments and monitor test results.
• Review preoperative surgical clearance requests with providers and triage calls as needed.
