Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs, plans nursing care, and ensures continuity of patient care.
• Communicates with provider regarding patient needs, nursing assessments, and recommendations.
• Takes direction and initiates actions that will allow seamless patient care.
• Collects and documents current health status and medical history and assists in scheduling patient office visits.
• Reviews patient records and discusses outstanding issues with provider.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Under direction of provider, calls in prescription medications to pharmacies and observes patient’s response.
• Documents accurate ongoing assessment of patient’s needs, problems, progress toward goals, and response to care.
• Triages telephone calls and initiates appropriate nursing intervention promptly and professionally.
• Assists patients and family members with concern and empathy; respects confidentiality and privacy.
