Job summary

Boston Children’s Hospital in Boston, MA, seeks an RN Case Manager in Cardiology to coordinate transitions from hospital to home or other settings based on patient need. The role requires clinical assessments and collaboration with the cardiology team to obtain necessary equipment and services for safe home care.

The position requires a Massachusetts RN license; Certified Case Manager is preferred or the willingness to obtain within two years.

Qualifications

• Bachelor’s degree in nursing required.
• Master’s in related healthcare field preferred.
• Minimum of 5 years nursing experience; pediatrics preferred.
• Massachusetts RN license required.
• Certified Case Manager preferred or willingness to obtain within 2 years.

Responsibilities

• Assessing, planning, organizing and overseeing daily clinical operations of care coordination and transitions.
• Knowledge of community health services, payer systems and post-acute care needs.
• Collaborative work with inpatient nursing teams and inpatient case managers.
• Integrate regulatory requirements, length of stay, funding sources and capacity management.
