Job summary

Infinite Medical P.C. in Chester, CT is seeking a Cardiology Nurse Practitioner or Physician Assistant to provide comprehensive heart care and collaborate with cardiologists to manage patients with cardiovascular conditions.

The role involves comprehensive assessments, medical management, diagnostics, and patient education. The position emphasizes regulatory compliance, CMS and NCQA guidelines, and close teamwork with the interdisciplinary staff to care for residents in long-term care

Qualifications

• Active and unrestricted NP/PA/Physician license
• DEA License
• CPR BCLS/BLS certified
• Minimum 6 months of experience in long-term care or a skilled nursing facility
• Master's Degree in Nursing
• Completed NP program from an accredited program
• Cardiology experience preferred (training provided)
• EMR documentation experience training provided

Responsibilities

• Perform comprehensive patient assessments including physical exams and histories
• Collaborate with cardiologists to develop and implement treatment plans
• Administer medications, monitor responses, and adjust plans as needed
• Conduct cardiac stress tests and EKGs
• Educate patients and families on cardiovascular health and adherence
• Provide counseling and emotional support to patients and families
• Ensure CMS Regulations and NCQA HEDIS alignment
• Maintain accurate medical records per OSHA and site safety standards
• Collaborate with interdisciplinary team
• Manage care for critically complex patients

Job Summary

As a Cardiology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with heart-related conditions. Responsibilities

• Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests
• Collaborate with cardiologists to develop and implement treatment plans for patients with cardiovascular conditions
• Administer medications, monitor patient responses, and adjust treatment plans as necessary
• Conduct cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures
• Educate patients and their families about cardiovascular health, preventive measures, and the importance of medication adherence
• Provide counseling and emotional support to patients and their families
• Knowledge of CMS Regulations and NCQA HEDIS Guidelines
• Document accurate medical records to ensure compliance with OSHA and Government regulations and perform duties in accordance with site quality and safety systems
• Collaborate with the interdisciplinary team
• Analyze and organize the care of critically complex patients

Requirements

• Minimum 6 months of experience in long-term care or a skilled nursing facility
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
• DEA License
• CPR BCLS/BLS certified
• Must hold a Master's Degree in Nursing
• Completed a degree from an accredited Nurse Practitioner program
• Preferred experience in cardiology: training provided
• Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
• Must have great organizational and time management skills.
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Carrying medical equipment. Lifting/moving patients.
