Heart Care in Frisco, TX is seeking an experienced Cardiology Nurse Practitioner or Physician Assistant (APP) to join our established outpatient cardiology practice. This role is 20-25 hours per week to start, with potential to expand based on performance and growth.

You will manage stable cardiovascular follow-up patients, optimize guideline-directed medical therapy, and support a patient-centered practice focused on Medicare Advantage populations. This position is strictly outpatient.

Qualifications

• Active Texas NP or PA license.
• Minimum 2 years of cardiology experience preferred.
• Strong comfort with outpatient chronic cardiovascular management.
• Experience with heart failure medication titration strongly preferred.
• Familiarity with Medicare Advantage documentation and risk-adjustment coding a plus.
• Ability to work independently with physician collaboration available on-site.

Responsibilities

• Evaluate and manage established cardiology follow-up patients in an outpatient setting
• Manage chronic cardiovascular conditions including heart failure, hypertension, coronary artery disease, arrhythmias, and hyperlipidemia
• Optimize and titrate guideline-directed medical therapy (GDMT)
• Order and interpret diagnostic testing including labs, EKGs, echocardiograms, and stress testing
• Remote patient monitoring of blood pressure and chronic care management
• Collaborate with cardiologists on complex cases and elevate care appropriately
• Provide patient education on medication adherence, sodium restriction, weight monitoring, and lifestyle modification
• Document encounters thoroughly and accurately within the EHR
• Ensure appropriate ICD-10 coding and risk-adjustment documentation for Medicare Advantage patients
• Assist with medication refills, inbox management, and care coordination

We are looking for a confident, independent clinician who is comfortable managing stable cardiovascular follow-up patients, optimizing guideline-directed medical therapy, and supporting a high-quality, patient-centered practice focused on Medicare Advantage populations.

This role is strictly outpatient. No hospital rounding.
