Qualifications

• 1 year experience required unless AVP level or above approved.

Responsibilities

• Promotes wellness and restores health through ambulatory care nursing.
• Assesses patients and develops individualized plans of care.
• Delivers patient education and coordinates care across the health care team.
• Maintains safety and compliance with standards of regulatory agencies.
• Participates in performance improvement activities.
