Job summary

Qualifications

• Minimum 5 years direct patient care experience.
• Graduation from an accredited school of nursing with clinical hours.
• Current RN license and required certifications at hire.

Responsibilities

• Perform the nursing process (assessment, diagnosis, outcomes, planning, implementation and evaluation) using evidence-based practice and individualized care.
• Provide relationship-based, patient-centered care respecting population-specific characteristics and cultural diversity.
• Partner with patients and families to set goals and keep patient goals at the center of the plan of care.
• Practice using current clinical standards and guidelines.
• Coordinate delivery and documentation of safe, high-quality care through interdisciplinary communication.
• Support performance improvement initiatives and evidence-based practice.

Associates degree in Nursing
