Job summary

Responsibilities

• Conducts cardiovascular disease risk factor evaluation.
• Develops a cardiovascular risk reduction plan individualized for each patient.
• Provides patient education and counseling in a broad spectrum of health-related areas.
• Guides the patient into a network of social support as needed.
• Assists the patient in developing strategies for behavioral modification.
• Provides exercise prescription and instruction.
• Instructs and leads group educational and exercise sessions.
• Monitors vital signs, telemetry, and symptoms.
• Collaborates with the team to ensure continuum of care during and after hospitalization.
• Participates in in-service education for staff and community outreach.
