Job summary

LiveWell Homecare Agency, LLC is seeking an experienced Registered Nurse to provide direct patient care for acute and intermediate cardiology conditions, including heart failure management and post-transplant care, with continuous telemetry monitoring.

You will assess, plan, and implement individualized care plans in collaboration with an interdisciplinary team, emphasizing patient safety, education, family-centered care, and ongoing professional development.

Qualifications

• RN licensure and BLS certification; ACLS within one year.
• Preferred: BSN or higher.
• Minimum 2 years of nursing experience.

Provides direct patient care for acute and intermediate cardiology conditions, including heart failure management, high-risk drips, post-transplant care, and continuous telemetry monitoring. The role involves critical assessment, planning, and implementation of individualized care plans in collaboration with an interdisciplinary team, emphasizing patient safety, education, and family-centered care.

Key responsibilities include:

• Promoting safe, quality, and regulatory compliant care
• Educating patients and families on illness management, medications, and post-treatment needs
• Assessing physical, psychosocial, and emotional needs, respecting patient values and cultural considerations
• Developing, implementing, and documenting care plans with team collaboration

Qualifications and skills required include:

• Nursing Diploma or Associate’s degree in Nursing (Bachelor’s preferred)
• RN licensure and BLS certification; ACLS certification within one year
