Job summary

Pulse Heart Institute, part of MultiCare, seeks a Nurse Navigator Acute Care to support cardiovascular patients across the acute care continuum. In this role you will function as a registered nurse, case manager, and patient/physician liaison, focusing on clinical quality and cost-effective outcomes for a defined caseload.

The position requires advanced cardiovascular knowledge, triage skills, and the ability to implement process improvements.

Qualifications

• Bachelor's degree in Nursing is required, or obtained within five years from initial date of hire.
• RN license in Washington State or MLS, BLS certification, and relevant area certification preferred.
• Three recent clinical experiences in area of specialty.

Responsibilities

• Provide comprehensive services across the acute care continuum as RN, case manager, and patient/physician liaison.
• Manage cardiovascular hospitalized patients, apply triage principles, and implement improvement processes.
• Collaborate with nursing staff, management, physicians, patients, families and community partners.
