Instituto Familiar de la Raza, Inc. is a multi-service community health and social service agency with an emphasis on serving the Chicano/Latino/Indígena community in San Francisco with a special focus on the diverse needs of the Mission District. Services include a wide range of mental health, HIV-related services, and social services, including health promotion, education, prevention, early intervention, case management, and psychological and psychiatric interventions, as well as cultural/social and spiritual re-enforcement. The agency serves children, youth, adults, and families. It has a rich history of working collaboratively with other agencies to meet the needs of the Chicano/Latino/Indígena community and other cultural/racial communities in San Francisco.
Job/Program Description
La Cultura Cura is looking for a highly motivated and community-driven bilingual (English/Spanish) professional to provide engagement, assess psycho-social needs, conduct follow-ups, and monitor service goals for participants of this program. We are seeking someone with experience in violence prevention and knowledge of trauma-informed care and restorative practices, along with the ability to work collaboratively with various system partners. La Cultura Cura offers a range of services and opportunities to youth and their families, with a particular emphasis on youth who have had contact with or are at high risk of entering the juvenile justice system. The program also provides youth and their families with care management and mental health services that are culturally responsive.
The Community Assessment & Referral Center (CARC) provides community-based diversion program services as a single point of entry for crisis intervention, intake/assessment, service integration, and referrals back to the community for arrested and detained youth. The goal of CARC is to assess the needs of young people who come into contact with law enforcement, connect them and their families to needed services or support, hold the youth accountable for their actions via restorative justice practices when applicable, and prevent recidivism.
In this position, providers will respond to youth ages 12 to 18 who have been cited or arrested by law enforcement in San Francisco, applying a juvenile justice model that connects youth to a Justice Services Care Coordinator (JSCC) for continued care management and additional service linkages from five core agencies. The navigator will work through a collaborative process and successfully negotiate with multi-disciplinary systems. This position is in-person on-site, in the community, and at the Juvenile Justice Center, with opportunities for remote work. It is primarily in-person, Tuesday through Saturday, from 10:00 a.m. to 6:00 p.m., with a rotating Saturday schedule.
Essential Job Duties are, but are not limited to the following:
- Respond to referring agents and engage youth in services with a rapid response.
- Engage and build rapport with youth at SFCARC, in the community, and those who are detained at the Juvenile Justice Center.
- Complete intake and assessment with youth who are arrested by law enforcement at SFCARC and at the Juvenile Justice Center.
- Provide system navigation, service connections, and coordination with the five JSCC providers within a 24–72-hour timeframe and ongoing as needed.
- Collaborate with youth, their family members/legal guardians, and JSCC providers to ensure successful navigation of the juvenile justice system.
- Conduct school, home, and community visits as needed for linkage and monitoring of connections.
- Attend juvenile court proceedings and meetings with Probation Officers and Public Defenders.
- Participate in collaborative meetings, inter-agency trainings and meetings, violence prevention activities, and other events as requested.
- Attend all program-specific and agency (IFR and CARC) required meetings.
- Enter all encounter/contact documentation and maintain client files in a timely and organized manner in Salesforce, CMS databases, or other data tracking systems.
- Manage an active caseload of 8–10 youth.
- Complete monthly reports for Program Managers and funding sources as needed.
- Participate in and engage with personal and professional development training focused on best practices in diversion, youth development, and related themes.
- Attend and participate in supervision and client review conferencing meetings.
- Perform other duties as assigned by the supervisor.
REQUIRED SKILLS:
- Bilingual in English and Spanish, fluent in both languages.
- At least two years of experience providing services to youth, families, and care management functions/interventions or other highly qualified interventions with adolescents.
- Demonstrated cultural competency in addressing the diversity of the Latino community, including gender, race, language, gender identification, sexual orientation, and socio-economic status.
- Excellent critical thinking, problem-solving, oral and written communication skills, and organization.
- Ability to follow protocols and keep client-sensitive information confidential, maintaining ethical boundaries and professional conduct.
- Ability to work a flexible schedule at multiple locations, being mobile to provide services.
- Ability to manage multiple tasks, work independently, as well as work cohesively in a culturally diverse team environment.
- Experience with collaborative and care coordination work.
PREFERRED SKILLS:
- Bi-/Multicultural competency.
- Experience in a non-profit community agency and knowledge of community resources serving adolescents in the San Francisco Bay Area.
- Ability to work in a crisis, fast-paced, and collaborative environment.
EDUCATION, CERTIFICATION AND LICENSES
- BA degree in Social Work, Criminal Justice, Psychology, Ethnic Studies, Human/Social Services, or a related field.
- A combination of relevant work, education, and training in the areas of social welfare, youth development, and/or community advocacy can be considered.
- Valid driver’s license.
CONDITIONS OF CONTINUED EMPLOYMENT
- Must submit to a tuberculosis test annually
- Must pass the Department of Justice Fingerprint screening
- Fully Vaccinated for COVID-19
Compensation And Benefits²
Salary Range: This is a full-time position with benefits, with an hourly salary range of $27.08 to $31.00, commensurate with work experience, qualifications, and education.
All full-time positions are based on a minimum of thirty-five (35) hours per week. This is a full-time, non-exempt position at 35 hours per week. The position is eligible to participate in medical, dental, vision, and 403(b) plans; earn vacation, sick leave, and paid holidays; receive two wellness days (birthday and hire date); and access ongoing opportunities for learning.

- Ability to use public and/or private modes of transportation; duties are office-based, but travel to various off-site venues is required.
- Must be able to work outdoors in various types of weather conditions.
- On occasion, must lift up to 35 lbs.
