RHA Health Services, LLC is seeking a CAP Nurse Case Manager (Part-Time RN) to join our team in Watauga, Avery, and Burke counties, North Carolina. This role focuses on providing community-based case management for medically fragile children, coordinating with RNs to ensure comprehensive, high-quality care in home settings.

The ideal candidate collaborates with an interdisciplinary team, supports person-centered planning, and helps families access essential services.

Qualifications

• Bachelor’s degree in social work or related human services field required.
• Directly related experience includes case management, assessment, referral, supportive counseling, intervention, psychosocial therapy and treatment planning.
• Ability to coordinate care with RN and interdisciplinary team to support community-based services.

Responsibilities

• Provide basic case management services for medically fragile children in their primary residence.
• Coordinate services with RN to ensure quality care.
• Engage families to develop person-centered plans and coordinate supports.
• Work with the Community Alternative Program for Children to prevent institutionalization.
• Collaborate with an interdisciplinary team to respond to crises and adjust plans as needed.

The Community Alternative Program for Children is a Community Based Services Waiver designed to provide an alternative placement in a nursing facility for beneficiaries who prefer to remain in his or her primary private residences and would be at risk of institutionalization without these services. We work with individuals to establish the person-centered plan and to coordinate care and services including personal care, waiver supplies, home modifications and educational support.

Requirements

• Bachelor’s degree in social work from an accredited school of social workOR
• Bachelor’s degree in a human services field from an accredited college or university and one year directly related experienceOR
• Bachelor’s degree from an accredited college or university and two years directly related experience.

“Directly related experience” is defined as human services experience in the areas of case management, assessment, and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.
