Foundations Health seeks a licensed Chemical Dependency Counselor (CADC) to serve at the High Point, NC opioid clinic. You will provide individual, group, and family counseling, complete psychosocial assessments, and set treatment objectives within OTP protocols. Qualifications

• High School Diploma required.
• CADC or CADC-I certification required.

Responsibilities

• Provides individual and group counseling based on the Client’s PCP.
• Provides substance use, health and community services education.
• Provides coordination and consultation with medical, clinical, familial, and ancillary relevant parties with Client consent.
• Ensures linkage to the most clinically appropriate and effective services along with arranging psychological and psychiatric evaluations.
• Develops individualized treatment plans and ensures implementation.
• Other duties as assigned.
