Job summary

Morris County Aftercare Center is seeking a licensed CADC counselor to provide individual and group counseling for clients on medically-assisted treatment. The role includes intake, assessment, case management, and discharge planning, with attention to licensure standards and client-centered care.

Responsibilities include developing treatment plans, facilitating Cognitive Behavioral Intensive Outpatient groups, and coordinating referrals and community services.

Qualifications

• Must hold an active CADC license.

Responsibilities

• Provide intake, assessment, and case management for clients on medically-assisted treatment.
• Monitor program eligibility and coordinate services; maintain care plans and notes.
• Offer information and referrals to community and agency services as needed.
• Participate in case conferences and Multidisciplinary Team meetings; discharge planning.
• Maintain client charts per licensure standards; ensure court stipulations are met.
• Manage a caseload of at least 50 clients; ensure documentation is complete.

General Description CADC

To provide individual and group counseling for the addicted population who are on medically-assisted treatment. Intake, assessment and case management services, ongoing assessment for program eligibility, monitoring all aspects of care and client services, and psycho-educational classes may be included at discretion of Clinical Director.

Job Requirements (includes but may not be limited to)

• Must have an active/current CADC license
• Knowledgeable in the drug and alcohol field
• Have a positive attitude with clients served
• Maintain professionalism and have a constructive approach to problem-solving
• Providing alcohol and substance abuse counseling in individual and group formats
• Facilitate required Cognitive Behavioral Intensive Outpatient groups and classes
• Develop a comprehensive treatment plan that meets the needs of the client

NJSAMS intake assessment

• Provide the client with information on community services and agency services which will address their needs and make the appropriate referrals on behalf of the client
• Utilization of case management skills
• Participates in case conference with clients and present cases at the Multidisciplinary Team meetings
• Discharge planning
• Maintain client chart according to licensure standards
• Ensure all services are supported by care plan, and case note documentation
• Maintain a caseload of at least 50 clients
• Ensure that all court stipulations are met by each client assigned to the caseload
• Ensure that all the duties of this position are performed in accordance with licensure standards

Qualifications/Skill Requirements

• CADC license
• A valid driver's license
• Working knowledge of NJSAMS preferred
• Good organizational skills
